Student Application Instructions

Please scroll down this page to check whether a class has closed before sending in your application.

1.  Download and completely fill out the form.  You must SIGN the form.

2.  Be sure to indicate which academy you are enrolling your student in. The demand for summer academies has been so strong, students will be limited to one academy per summer.

3.  Scan/email your application to the respective STARBASE location.  Email information is located on the student permission form.

4.  We will email you once your application is accepted. It may take up to several days to acknowledge that we have received your application. We will then contact you within two weeks to let you know if your child has a seat in the academy.

5.  Please consider a donation to help support our program. Please see the Donate button on the home page for access to each site’s form.

6. Priority will be given to children of a parent/guardian that is either active duty, reserve or retired military, as well as students who have not previously attended STARBASE.
